Providence ElderPlace PACE Program to open first center in Snohomish County

Residents offered healthcare, social services, and transportation

Providence ElderPlace, a care system for older adults that combines healthcare and social services in one place, will open its first Program of All-inclusive Care for the Elderly (PACE) in Snohomish County on April 1, 2022. This innovative program offers medical care, social services, and transportation to older adults who wish to continue to live in the community, thus avoiding or minimizing stays in hospitals and skilled nursing facilities.

Providence ElderPlace in June 2020 first expanded services to serve residents in a limited area of South Snohomish County that includes Lynnwood, Mountlake Terrace, Brier, Edmonds and the Snohomish County parts of Woodinville and Bothell. The new Snohomish County center is located at 1615 75th St. SW, #220, Everett, Wash. Eligible residents in the new service area that includes Everett, Lynnwood, Edmonds, Mountlake Terrace, Snohomish, Monroe, Mukilteo, Lake Stevens, Marysville and Tulalip, can start to enroll beginning on April 1, 2022.

Providence ElderPlace provides transportation from each individual’s residence to the ElderPlace center, where they receive medical and social services, including adult day health activities.

“We’ve been looking forward to expanding our service area to Snohomish County for years,” said Providence ElderPlace Executive Director Susan Tuller. “We look forward to bringing this program to those who will be able to benefit from our healthcare and social services. Because we offer wrap-around care, those who enroll with us are able to continue to live in the community, rather than in a nursing home.”

Providence ElderPlace, which serves more than 1,040 seniors in its system, continues to expand its service area to meet the growing needs of older adults who want to be able to age in place in their communities of choice. Providence ElderPlace runs five full-service centers in King County and one in Spokane. Each includes a full medical clinic with a geriatrician, registered nurses, physical and occupational therapists, in addition to offering social programs. New enrollments are welcome.

Providence ElderPlace’s PACE program in Washington currently provides:

  • Primary and specialty medical care
  • Nursing care
  • Occupational, physical and speech therapies
  • Social workers
  • Family and caregiver support
  • In-home personal care services
  • Prescriptions and over-the-counter medications
  • Adult Day Health

Eligible individuals must be age 55 and over, in need of assistance with activities of daily living as determined by the state of Washington, and be able to live safely in a community setting.

Providence ElderPlace PACE is an innovative program of healthcare and social services for older adults that is part of the national Program of All-inclusive Care for the Elderly (PACE). The non-profit Providence ElderPlace keeps older adults as healthy as possible in the community by providing comprehensive healthcare and social services including: primary and specialty medical care, a day health program, social work services, rehabilitation, and more. Participants can attend a Providence ElderPlace center on a regular basis and transportation is provided. The Providence ElderPlace team of health care and social service professionals and affiliates provide comprehensive integrated care to participants.

In addition to the new center in Snohomish County, Providence ElderPlace has centers in South Seattle’s Rainier Valley, at Providence Mount St. Vincent in West Seattle, in Redmond, at two locations in Kent and one in Spokane. Participants attend the Providence ElderPlace centers on a regular basis and transportation is provided.
